Knee replacement alternatives do different jobs

This comparison covers choices that may help before or instead of surgery. Some build leg strength, while others move pressure or calm soreness. Each one acts on a different part of the knee problem. Your exam, daily limits and health risks help narrow the choices.

Daily movement and strength reduce strain

Strength in the thighs and hips steadies the knee with each step. Regular practice can make standing, stairs and walking feel steadier. Start below the amount that causes a strong ache later. Then add time or effort slowly as your knee settles.

Body weight can add pressure, but it doesn't cause every sore knee. Smaller changes may help before your weight changes much. Try shorter walks, level ground and rest between harder days. A cane can share some of the load while you move.

An unloader knee brace moves pressure away

An unloader knee brace is made for wear that is worse on one side. It shifts some pressure toward the other side during standing and walking. The brace must fit well enough for you to use it. Ask for an adjustment if it rubs, slips or doesn't help.

A brace won't suit every kind of knee wear. The exam and X-ray can show whether pressure sits on the sore side. First wear it during a short, familiar activity. Check your skin afterward and notice whether walking feels easier.

genicular artery embolization blocks small knee blood vessels, but benefit is uncertain

Genicular means around the knee, and embolization means blocking a blood vessel. The procedure blocks small arteries feeding irritated knee tissue. Less blood then reaches that tissue, with the aim of easing soreness. That idea hasn't led to a clear benefit in human testing.

In one human trial, people improved after the real procedure and after a sham. A sham is a convincing imitation that doesn't block those small arteries. Since the real procedure wasn't clearly better, cost and risk deserve careful thought. Ask how any proposed procedure would improve your walking or sleep.

Alternatives to knee replacement surgery have limits

Surgery can make sense when severe soreness and lost function remain after sound care. Replacement changes damaged joint surfaces, while most other choices don't. The operation also brings recovery work and risks worth discussing. Waiting makes sense only while the knee and daily life remain manageable.

Before deciding, record your walking distance and nights disturbed by soreness. Note any falls, buckling or days when the knee stops normal tasks. Topical NSAIDs are strongly recommended for knee OA (Level 1A). The 2019 ACR/Arthritis Foundation guideline makes STRONG recommendations for exercise, weight loss in people with knee and/or hip OA who are overweight or obese, self-efficacy and self-management programs, tai chi, cane use, tibiofemoral bracing for tibiofemoral knee OA, topical NSAIDs for knee OA, oral NSAIDs, and intra-articular glucocorticoid injections for knee OA. Radiofrequency ablation for knee OA, acupuncture, thermal modalities, acetaminophen, duloxetine and tramadol are only CONDITIONAL recommendations.

Pooling the OAI and MOST cohorts (8,002 participants followed up to 8 years, 3,417 classifiable knees), validated appropriateness criteria classified only 290 knees (8%) as receiving a TIMELY total knee replacement, 2,833 knees (83%) as potentially appropriate but not replaced more than 2 years after replacement became appropriate, and 294 knees (9% of all knees, 26% of the 1,114 replacements actually performed) as PREMATURE. Of the potentially-appropriate-but-not-replaced knees, 1,204 (42.5%) had severe symptoms.

  4. A systematic review and meta-analysis of 169 randomised trials (21,163 participants) of viscosupplementation for knee OA found clear evidence of small-study effects and publication bias. The prespecified main analysis, restricted to 24 large placebo-controlled trials with at least 100 participants per group (8,997 randomised), found a pain reduction of SMD -0.08 (95% CI -0.15 to -0.02) - the confidence interval excluding the prespecified minimal clinically important difference of -0.37.

  5. The RESTORE trial randomised 288 community-based participants aged 50+ with symptomatic medial knee OA (Kellgren-Lawrence grade 2 or 3) to three weekly intra-articular injections of leukocyte-poor PRP or saline placebo, with participants, injectors and assessors all blinded. 93% completed the 12-month follow-up. PRP did not produce a clinically meaningful improvement in knee pain over placebo, and did not slow medial tibial cartilage volume loss on MRI.

  6. A meta-analysis of 16 randomised trials (807 participants) of intra-articular mesenchymal stem cells for chronic knee pain from osteoarthritis found that at 3-6 months MSC therapy probably produces little to no difference in pain (WMD -0.74cm on a 10cm VAS, 95% CI -1.16 to -0.33, against a minimally important difference of 1.5cm) or physical function, both moderate certainty; at 12 months, probably little to no difference in pain. MSC therapy may increase the risk of any adverse event (RR 2.67, 95% CI 1.19 to 5.99) and of knee pain and swelling (RR 1.58, 95% CI 1.04 to 2.38).

  7. In the only randomised controlled trial to compare total knee replacement directly with non-surgical treatment in patients already eligible for surgery (n=100), the replacement group improved more at 12 months than the non-surgical group (KOOS4 32.5 vs 16.0; adjusted mean difference 15.8, 95% CI 10.0 to 21.5) but had four times the serious adverse events (24 vs 6, P=0.005). Only 13 of 50 patients (26%) assigned to non-surgical treatment alone had undergone knee replacement by 12 months.

  8. At 2 years across two parallel randomised trials (200 patients, mean age 66), total knee replacement plus non-surgical treatment beat non-surgical treatment alone by 18.3 KOOS points (95% CI 11.3 to 25.3), and non-surgical treatment in turn beat written advice by 7.0 points (95% CI 0.4 to 13.5). Among patients eligible for replacement, 16 of 50 (32%) in the non-surgical group had surgery within 2 years - meaning two out of three delayed it for at least two years.

  9. A 2026 Cochrane review searching from 2010 to January 2025 found only ONE randomised trial (100 participants, Denmark) comparing knee arthroplasty with non-surgical treatment. Low-certainty evidence indicated TKA may reduce pain at one year by a clinically important margin (MD 17.60, 95% CI 8.25 to 26.95) and may improve function by an amount that might not be clinically important (MD 12.40), with probably no clinically important difference in health-related quality of life.
